1. Application information: full name, nationality and address of the applicant and designer;

2. Design pictures or photographs: the applicant shall submit a three-dimensional view (unfolded view) and a six-sided view (I. e., a front view, a rear view, a left view, a right view, a top view and a bottom view). The dimensional proportions of the seven-sided view must be consistent;

3. Brief introduction of the design and claims;

4. If the right of priority is claimed, the notice of acceptance of the earlier application and the priority supporting documents of the earlier application must also be provided;

5. A signed and notarized power of attorney.
